Your Privacy Matters 

At CWT Behavioral Health, we care about your privacy. This notice explains how we use and share your health information, and what rights you have. We follow federal and state laws that protect your personal health information (PHI). If we make changes to this notice, we will post the new version in our office and on our website. The new notice will apply to all the information we have about you. 

Our Promise to You 

We promise to: 

  • Keep your health information private and safe. 

  • Give you this notice to explain our privacy practices. 

  • Follow the rules in this notice while it is in effect. 

How We Use and Share Your Health Information 

We use your health information to help take care of you, get paid for services, and run our business properly. Here are some examples: 

  • Treatment: We use your information to give you care or talk with other professionals helping with your care. For example, your therapist might talk with your doctor or another therapist to make sure you get the right treatment. 

  • Payment: We may use or share your information to bill insurance companies or to collect payment for services. 

  • Health Care Operations: We may use your information to check how our services are working, train staff, or improve quality of care. 

Other Times We May Share Your Information 

Sometimes, the law allows or requires us to share information without your permission. Examples include: 

  • When the law requires it. 

  • To report suspected child, elder, or dependent adult abuse or neglect. 

  • To help stop or prevent a serious threat to someone’s health or safety. 

  • For health audits or investigations. 

  • If ordered by a court or during legal cases. 

  • For law enforcement (for example, if a crime happens at our office). 

  • To coroners or medical examiners doing their lawful duties.

  • For workers’ compensation cases.

If You Want to Limit Sharing 

You can ask us to limit how we use or share your information. We may not always be able to agree, but we will do our best. If you pay for a service completely out-of-pocket, you can ask us not to share that service information with your health plan. We will honor that request. You can also tell us the best way to contact you, like at home, by mail, or by email and we’ll do our best to follow your wishes. 

Your Rights 

You have the right to: 

  1. See and Get a Copy of Your Record: You can ask for an electronic or paper copy of your health record. We will provide it within 30 days of your written request. There may be a small fee to cover costs. 

  2. Ask Us to Fix Your Record: If something is missing or wrong in your record, you can ask us to fix it. We may say no, but we’ll tell you why in writing within 60 days. 

  3. Get a List of Who We Shared Information With: You can ask for a list of times we shared your information (except for treatment, payment, or health care operations). We’ll give this to you for the past 6 years, for free once a year. 

  4. Ask for a Paper or Email Copy of This Notice: You can always get a paper copy of this Privacy Policy, even if you got it electronically. 

  5. Ask for Restrictions: You can ask us not to share certain information, but we might not be able to agree if it affects your care. 

  6. Ask for Confidential Communication: You can ask us to contact you in a special way (like sending mail to a different address). 

When You Can Object 

We may share your health information with family, friends, or others involved in your care if you agree. If you can’t tell us your wishes (for example, in an emergency), we may share information if it’s in your best interest.
